Harris HospisCare provides a home care service that delivers care to patients at home and in clinic based appointments at the hospice.
Each patient has access to the multi-professional team of doctors, nurses, a social worker, a welfare officer, complementary therapists, chaplain and arts therapists.
GPs retain clinical responsibility for home care patients and the Harris HospisCare clinical nurse specialists (CNSs) work in partnership with both the GP and the district nurse. CNSs also contribute to GP practice-based Gold Standard Framework (GSF) meetings.
There is a 24 hours a day and seven days a week out-of-hours advice and visiting service for patients and carers at home. Healthcare professionals may also ring at any time to discuss patients, including those who are not known to Harris.
The Caritas Centre at the hospice incorporates a social area where patients can relax. They can join in the creative activities organised in the day unit including painting, pottery and music.
